    • Case Management – Support patient care plans and coordinate follow-ups based on provider instructions.

    • Advance Follow-Up Tasks – Manage Remote Patient Monitoring (RPM) and Chronic Care Management (CCM), ensuring ongoing patient engagement and health tracking.
